Current release version (3.1.4 build 4) I'm seeing this often but not quite all the time:
Look in the circles. Same channels starting an effect. The table view is showing the effect number in blue (correctly), the tombstone view is showing the effect number in purple/tracking even though the effect is starting here (which is incorrect).
I have a suspicion it's something to do with how the effect is started, ie. in live vs in blind, or by copying a channel that's already running the effect vs telling a channel to run the effect. I can't pin it down but it is happening a lot and is frustrating, particularly if you only have a tombstone view open.
And no, these are not multi-cell fixtures or anything odd like that, just standard Mac Encores.
