This normally means that the head card or box card might have failed. Please try swapping the head card with another unit and see if the problem follows the head card. If it does not, then try swapping the box card as well.

The USB chip for the motion system is located on the head card. The USB INIT ERROR you are seeing is because the server computer cannot communicate with the the USB chip on the head card.
Sometimes, the head card will have a failure and the USB chip will be inactive. But, other times, if the box card is having problems, it will not allow communications up to the head card.
